G. L. c. 66 § 10(b); see also 950 C.M.R. 32.08(2). Specifically, Mr. Murray requested a "reimbursement form" issued to a specifically identified individual. Having received no response, Mr. Murray filed an appeal with this office. Subsequent to a telephone conversation between you and a member of my staff it was determined that the Old Colony Correctional Center has not received any recent requests made by Mr. Murray.
