Please be advised that a request for appeals to the Supervisor of Records shall include a complete copy of the letter by which the request was made, including in the case of electronic communications all header information indicating time, date, subject, sender and recipient email addresses; and a complete copy of all written responses associated with requests subject to the petition for appeal, including in the case of electronic communications all header information indicating time, date, subject, [and] sender and recipient email addresses. Please note that you also did not include the original letter of request you sent to the Department. See id. As no such copy was provided, I am unable to assist you in this matter.
