Please be advised that a request for an appeal to the Supervisor of Records shall include a complete copy of the letter by which the request was ·made, including in the case of electronic communications all header information indicating time, date, subject, sender and recipient email addresses; and a complete copy of all written responses associated with requests subject to the petition for appeal, including in the case of electronic communications all header information indicating time, date, subject, [a nd] sender and recipient email addresses. Please note that you did not include the original public records request you made to the Department, only the letter of response. See id. Accordingly, I am unable to assist you in this matter.
