G. L. c. 66, § 10A; see also 950 C.M.R. 32.08(1). On September 17, 2021, Ms. Wellington requested an identified Serious Reportable Event report. Unsatisfied with the Department’s response, Ms. Wellington petitioned this office and this appeal, SPR21/2492, was opened as a result. Subsequently, I learned that the Department responded to Ms. Wellington’s request with a further response sent on October 4, 2021. Where the basis of this appeal was opened as a result of the Department’s September 21st response, I will now consider this administrative appeal closed.
