G. L. c. 66, §10A; see also 950 C.M.R. 32.08(1). The requirements for an appeal are follows: Such appeal shall be in writing, and all appeals to the Supervisor must include a copy of the original request, any responses by the Records Access Officer, and a statement indicating the reason for the appeal. 950 C.M.R. 32.08(1)(g). In Mr. Mazurczyk’s letter of appeal to this office, the required copy of the original request was not provided. Given that a copy of the original request was not included, I am unable to process the appeal at this time.
