See G. L. c. 66, § 10A; see also 950 C.M.R. 32.08(1). On October 25, 2024, Ms. Ibrahimi requested the following records: [1] Any incident reports and or any other documentation related to the death of [an identified individual] on September 27, 2018[;] [2] Any incident reports and or any other documentation related to the death of [an identified individual] on May 7, 2017[;] [3] Any incident reports and or any other documentation related to the death of [an identified individual] on May 11, 2017[;] [4] Any incident reports and or any other documentation related to the death of [an identified individual] on November 7, 2017. In making the request, Ms. Ibrahimi specified that she sought the responsive records, “electronically, in accessible format, including for screen readers” and requested that the Department “provide xls or xlsm files when relevant.” The Department responded on November 8, 2024. Unsatisfied with the Department’s response, Ms. Ibrahimi petitioned this office and this appeal, SPR24/3162, was opened as a result. Subsequent to the opening of this appeal, I learned that the Department provided a supplemental response by email to Ms. Ibrahimi and this office on November 29, 2024. As this appeal was opened based on the Department’s November 8th response, I will now consider this administrative appeal closed. If unresolved issues remain, Ms. Ibrahimi may further appeal the substantive nature of the Department’s November 29th response within ninety (90) days. See 950 C.M.R. 32.08(1).
