On July 21, 2025, Ms. Kadis requested “any and all records pertaining to the internal investigation conducted by the Bridgewater Police Department regarding the death or welfare-related response involving an [identified individual] during the year 2025.” Specifically, Ms. Kadis sought: [1] A full copy of the internal affairs or internal investigation report[;] [2] Any supporting documents, audio, video, communications, emails, or officer statements referenced or included in the investigation[;] [3] Any determinations, findings, or conclusions made by the department or its leadership[;] [4] Any records identifying the officers involved, their roles, and any subsequent actions taken[;] [5] The name and title of the official(s) who conducted and/or reviewed the investigation[;] Claiming to not yet have received responsive records, Ms. Kadis petitioned this office and this appeal, SPR25/2509, was opened as a result. Despite being notified of the opening of this appeal, it remains unclear whether the Department has provided a response.
